First responders—including firefighters, paramedics, emergency medical technicians, military rescue units, and law enforcement personnel—routinely encounter traumatic events that pose significant psychological risks. Prolonged exposure to high-stress environments, repeated witnessing of life-threatening situations, and workplace culture discouraging emotional expression contribute to elevated rates of Post-Traumatic Stress Disorder (PTSD) compared to the general population. This paper synthesizes clinical research on PTSD manifestation among first responders, explores neurological and psychosocial etiologies, examines barriers to treatment, and proposes the Occupational Trauma Response Framework (OTRF) for prevention, early detection, and long-term rehabilitation. Policy recommendations include mandatory mental-health screening, trauma-informed leadership protocols, peer-support systems, resilience training, and legislative protections.
